Job description
The PCIL Transition team is looking for a highly organized, self-motivated, outgoing people person to provide needed services to youth and adults living with a disability, who have a goal of employment. Career Transition Counselor will offer services related to advocacy, counseling on enrollment of post-secondary services, job exploration counseling, workplace readiness training, and work-based learning experiences within Mercer County. You will be working with youth and adults with a variety of disabilities such as developmental disabilities, autism, mental health, ADHD, and learning disabilities to name a few. Our goal is to help prepare these individuals to identify career paths, advocate for their rights, explore hands-on opportunities, and succeed in the employment world. This can be either a full-time or part-time position.

Essential Duties:
· Provide identified topics of need to individuals either through small groups or on a one-to-one basis.
· Responsible for developing curriculum topics outlined by the funder and disseminating curriculum within individual or group sessions.
· Assist with individual goal development for each individual.
· Complete all necessary reporting, assessments, and assignments outlined through the funder.
· Provide progress notes on each individual.
· Develop attendance sheets identifying topics discussed.
· Complete PCIL data entry for each individual.
· Creating Independent Living Plans and adding goals to the PCIL database.
· Relationship building with families to create engagement and participation. · Develop partnerships with schools, community employers, non-profits, and other organizations to recruit guest speakers, work site visits, and job shadowing opportunities.
· Organize after-hour group meetings, if needed.
· Attend webinars, conferences, and training to enhance skills.
· Work with businesses to develop potential internship or volunteer opportunities. · Assists in providing information and referral to consumers, their families, and the general public under our core services.

Required Knowledge/Skills/Attributes:
· Ability to engage with individuals within group or individual sessions.
· Ability to successfully relate to and communicate with people with disabilities, their families, other organizations, agencies, volunteers, and consumers.
· Commitment to ensuring individuals and families are provided with the highest level of service to ensure they are well informed and supported in the achievement of their goals.
· Knowledge of programs and services, benefits, eligibility, and supports available to persons with disabilities, as needed.
· Experience using Windows editions 8-10 including expertise in utilizing: Office 365; Outlook; Zoom; Microsoft Word, Excel, PowerPoint, Publisher, etc.
· Strong team player willing to take direction, initiate business activities, and work with management staff and peers.
· Self-starter, able to work with minimal supervision/direction.
· Professional demeanor and presentation.
· Have strong and effective spoken and written communication skills.
· Creative thinker using sound judgment in workload coordination and in technical matters · Ability to organize work effectively and establish priorities.
· Ability to meet or exceed deadlines.
· Ability to make decisions and implement activities impacting the organization’s well-being.

Education and Experience:
· Minimum associate degree, bachelor’s degree preferred.
· Experience working in a classroom setting and/or job development preferred. · One year of experience working with people with disabilities (experience can be paid, volunteer, or personal).
· Fingerprinting, Federal and State background checks as required.
· Must have own transportation, a valid driver’s license, and insurance as travel in Mercer and/or Hunterdon Counties is required.

Working Conditions:
· On-site – Standard Work Week is 9:00 AM – 4:30 PM with a half-hour break (35 hours). · Requires flexibility to meet the requests to conduct programs within schools or individuals before or after standard hours.
· This position has no required physical elements needing special designation.

Benefits/Comments:
Full-time position, Monday through Friday. Professional development and continued learning opportunities are offered. Compensation: $38,000 - $45,000 per year
