Job Overview:
We are seeking a Support Coordination (SC) Supervisor to join our team. As an SC Supervisor, you will be responsible for program development and management, working with individuals with developmental disabilities, and providing support to Support Coordinators. This is a rewarding role that requires strong interpersonal skills and a passion for helping others.

Duties:

Supervise a Support Coordination team in conjunction with the Support Coordination Assistant Supervisor
Keep up to date with DDD requirements for documentation and writing for all plans;
Ensure current knowledge through DDD Policies and Procedures Manuals, DDD Updates, PCIL Plan Guidelines, DDD Best Practices Guides, DDD Update Webinars, among others;
Utilize DDD databases appropriately, including but not limited to iRecord, UpDoc, JIRA;
Review all annual documentation for Individual Service Plans and Person-Centered Planning Tool of all plans;
Review and submit plans through the iRecord system for DDD reviews as needed;
Ensure that services are provided in accordance with the service definitions and parameters outlined by the state;
Ensure all required documentation is in accordance with DDD Policy and Procedure, ISP Plan Reviews: Guidance for SCA, and PCIL Plan Guidelines;
Provide a thorough review of all plans utilizing the required documentation noted above; complete the PCIL Plan Review Template to provide “kickback” documentation of all edits, provide explanations of requested edits and guidance as applicable to enhance the quality of each SC’s plan writing skills to ensure final approvals upholds the PCIL standards;
Approve plans accordingly against all DDD and PCIL requirements through the iRecord system;
Prioritize incoming plans for review based on level of priority/deadlines and needs of department;
Review and submit all forms DDD Support Coordination Supervisors oversee as described in the DDD Policy and Procedures Manuals – including, but not limited to: Seeking Out Support (SOS), Retroactive Change Requests (RCR), Intensive Case Management Requests (ICM), MLTSS transitions, and Unusual Incident Reports (UIR) to DDD;
Review with SC to discuss areas to improve when reviewing and submitting these documents as needed;
Guide the SCs through each of these processes and support them in discussions with DDD;
Follow-up on these documents and procedures with DDD and SCs to ensure procedures are followed appropriately;
Provide on-call assistance to SC based on agency on-call rotation and notifies appropriate SCAS;
Work in collaboration with the SC Supervisory (SCS) Team to update PCIL Plan Guidelines as per DDD standards as needed;
Maintain communication with SCS Team on training needs and areas of improvement for Support Coordinators
Review and submit Incident Reports (UIR) completed by Support Coordinators in accordance with PCIL Process for Incident Reporting with the PCIL Policy and Procedures Manual;
Meet with the SCS Team weekly;
Provide updates on plan submissions, MLTSS, UIRs, RCRs, ICMs, SOSs, SC headlines and issues, new assignments updates, and any other pertinent information to the administration of the SC Team;
Maintain state and federal documentation requirements and deadlines.
Hours of Work:

Full-time, Flexibility between Mercer and Hunterdon office locations, and hybrid work-from-home option available for discussion after 90 days probationary period.

Monday through Friday, 9:00 AM – 4:30 PM; half-hour lunch break (35 hours).

Flexibility is needed for Support Coordinator needs, state deadlines, and department needs.

Required Education and Experience:

Minimum of Bachelor’s degree
2 years’ experience as a Support Coordinator
Fingerprinting, Federal and State background checks required

PCIL has been empowering the lives of people living with disabilities since 1996. We offer our four core services: information and referral, advocacy, peer support, and independent living skills. Programs available at PCIL encompass our mission to advance the independent living and rights of people with disabilities. Services are available in Mercer and Hunterdon counties, with Support Coordination also available in Somerset County.
